Introduction: Pressure lesions (PPL) correspond to damage to the skin or covert soft tissue, usually in areas of bony prominence, or associated with the use of a medical device or other type of artifact. Therefore, nurses should develop their practice, based on scientific evidence and prioritize the prevention of skin lesion formation. Objective: To report the experience of nursing students on the development of actions to promote safe care related to the prevention of PPL in patients admitted to a pediatric public hospital in Bahia. Methodology: This is a descriptive study, of the type of experience report, conducted by six nursing students in a pediatric state hospital located in the state of Bahia, in the period of November 2022. Main results: It was noticed that there are nursing professionals who have an interest in obtaining more knowledge about the subject, but there are still gaps in knowledge on the part of the team on some subjects addressed, such as the Braden scale and the use of the egg box mattress for all age groups. It was possible to notice that the steps to prevent pressure injury are not met by the team and this can hinder the prevention of injuries effectively. Conclusion: It is recommended, therefore, that the care units, together with their respective coordinators, carry out continuous actions of continuing education, in order to contribute to a better professional qualification, promoting, above all, patient safety.

Introduction: The organization of pediatric services is essential in the current process of confronting the COVID-19 pandemic. Objective: To describe, from a nursing perspective, the social representation on management in the organization of pediatric hospital services for the care of patients suspected of COVID-19. Methods: A qualitative study of transcendental phenomenological methodological orientation was carried out at Octavio de la Concepción de la Pedraja Pediatric Hospital from Holguín, Cuba, between January and April 2021. The knowledge acquired from the experience of nurses, five supervisors, three area heads and eight ward chiefs, selected by nonprobabilistic sampling, was integrated. The following phases were used: descriptive, structural and discussion. The descriptive phase consisted of three stages: choice of technique, semistructured interview and elaboration of the description. The structural phase had the following stages: reading, determination of the central topic and expression in scientific language. In the results discussion phase, the results were related to the conclusions and findings of researchers to understand the possible differences or similarities for further enhancement. Results: Four categories emerged from the data analysis: design of nursing care protocols for COVID-19 positive and suspect patients; reorganization, recruitment and training of personnel; organization of working groups according to clinical picture and rapid test or PCR results; and general measures. Conclusions: The organization of nursing care services by groups avoided the collapse of the nursing human resource and guaranteed the availability of beds, as well as staff own safety and patient safety, to minimize in-hospital transmissibility(AU)

AND "enfermería", "cultural congruent care" AND "nursing" OR "cultural care" AND "nursing", cuidado cultural.

Introduction: In the context of a global and diverse society, culturally congruent nursing care is an urgent need. Accelerated migration processes, changes in the global economy and universal recognition of human rights unequivocally evidence this need. Objective: To identify evidence on the benefits of culturally congruent nursing care as part of patient well-being and satisfaction. Methods: Integrative systematic review of original studies published from 2000 to 2019 in Spanish and English. Seven databases were used: SciELO, Lilacs, EBSCO Host, Dialnet, DOAJ, Redalyc and Pubmed. The following search strategy was designed: "cuidado cultural" AND "enfermería", "cultural congruent care" AND "nursing" OR "cultural care" AND "nursing", cuidado cultural [cultural care]. Fourteen articles were selected and processed through content analysis. Conclusions: The selected articles permitted to identify the benefits of culturally congruent nursing care observed in different groups such as adults, aged adults and their caregivers, breastfeeding mothers, school children and adolescents. The benefits of cultural care were evidenced by addressing phenomenons such as family functionality, caregiver overload, promotion of breastfeeding, coping with stress, anxiety and depression, as well as quality of life in breast cancer survivors. Culturally congruent nursing care evidenced the development of a better nurse-patient relationship, as well as greater patient and family satisfaction(AU)

RESUMO

BACKGROUND Albumin level does not precisely reflect nutritional status. We aimed to investigate the impact of a nutrition intervention on hemodialysis patients by use of fluorescence-based plasma albumin (FPA) detection. MATERIAL AND METHODS Eighty patients underwent maintenance hemodialysis for more than half a year and had a mean albumin <3.5 g/dL for over 3 months. The subjects were randomly divided into either a Control Group (CG) or an Intervention Group (IG). The IG received nutritional supplementation, and the CG group received routine nutritional support for 12 months. FPA and plasma albumin (PA) concentrations were measured. The fluorescence probe 1,3-Dichloro-7-hydroxy-9,9-dimethyl-2(9H)-acridone methyl biphenyl benzoate was used in FPA detection. Quality of life was estimated using WHOQOL-BREF (Quality of Life Scale developed through the World Health Organization), the 36-Item Short-Form Survey (SF-36), and the 6-minute walking test (6MWT). RESULTS After a 6-month and a 12-month intervention, PA and FPA concentrations increased, and the increase in FPA concentration was higher than that of PA in the IG group (P<0.05). Comparatively, the parameters of quality of life and 6MWT were improved in the IG group (P<0.05) but there were only minor changes in the CG group (P>0.05). There is an obvious association between the changes in FPA concentration and the parameters of quality of life and 6MWT but not PA. CONCLUSIONS Use of the fluorescence probe improves the detection sensitivity of plasma albumin and provides a potential method to assess clinical outcomes in hemodialysis patients.

Introduction: Breastfeeding is a public health concern worldwide. However, the percentage of newborns worldwide who are exclusively breastfed has decreased in recent years. Objective: To describe the nursing cares that strengthen breastfeeding in neonates attended in joint accommodation and intermediate care. Methods: Qualitative and descriptive study with an exploratory design based on Swanson's and Kolcaba's theory. It was carried out during 2018, in the joint accommodation and intermediate care areas of Regional Teaching Hospital of Trujillo. Fifteen care nurses and nursing interns participated. Data collection was nonrandom and carried out using the interview. Discourse analysis was carried out after literal transcription of the interviews. Results: Two categories were identified: "Ensuring care for breastfeeding" and "Promoting commitment to the family and the institution". Conclusions: Nursing cares consist in identifying care needs, facilitating breastfeeding through comfort, support and education for the mother. Likewise, it lies in the commitment to breastfeeding, through continuity of breastfeeding, with the consideration of beliefs; as well as promotion of family and institutional involvement and the application of current norms that strengthen breastfeeding(AU)

RESUMO

BACKGROUND: Traditional prenatal care includes up to 13 in person office visits, and the cost of this care is not well-described. Alternative models are being explored to better meet the needs of patients and providers. OB Nest is a telemedicine-enhanced program with a reduced frequency of in-person prenatal visits. The cost implications of connected care services added to prenatal care packages are unclear. METHODS: Using data from the OB Nest randomized, controlled trial we analyzed the provider and staff time associated with prenatal care in the traditional and OB Nest models. Fewer visits were required for OB Nest, but given the compensatory increase in connected care activity and supplies, the actual cost difference is not known. Nursing and provider staff time was prospectively recorded for all patients enrolled in the OB Nest clinical trial. Published 2015 national wages for healthcare workers were used to calculate the actual labor cost of providing either traditional or OB Nest prenatal care in 2015 US dollars. Overhead expenses and opportunity costs were not considered. RESULTS: Total provider cost was decreased caring for the OB Nest participants, but nursing cost was increased. OB Nest care required an average of 160.8 (+/- 45.0) minutes provider time and 237 (+/- 25.1) minutes nursing time, compared to 215.0 (+/- 71.6) and 99.6 (+/- 29.7) minutes for traditional prenatal care (P < 0.01). This translated into decreased provider cost and increased nursing cost (P < 0.01). Supply costs increased, travel costs declined, and overhead costs declined in the OB Nest model. CONCLUSIONS: In this trial, labor cost for OB Nest prenatal care was 34% higher than for traditional prenatal care. The increased cost is largely attributable to additional nursing connected care time, and in some practice settings may be offset by decreased overhead costs and increased provider billing opportunities. Future efforts will be focused on development of digital solutions for some routine nursing tasks to decrease the overall cost of the model. RESUMO

The development of care research and its teaching in the Faculty of Nursing at Universidad de Antioquia cannot be understood outside the context in which it emerges and from the trajectory of its leading players. This is how this article will present a synthesis of the future of research, with its principal milestones and events in institutional history, in dialogue with the trajectories of four teaching nurses, protagonists of key moments in said history and living testimony of 70 years of institutional life. This panorama seeks to constitute a referent for strategic reflection, which inspires new generations to meet the research challenges and teach care research.

RESUMO

BACKGROUND: Chronic conditions are a major contributor to the burden of disease worldwide. Using innovative digital health technologies, nurse scientists are well-positioned to lead efforts to reduce the burden of chronic conditions on individuals, communities, and systems. PURPOSE: The purpose of this review was to summarize the outcomes commonly measured in nurse-led digital health interventions that target chronic conditions and pose recommendations for the education of future nurse scientists to lead these studies. METHODS: A rapid review of the literature was completed using CINAHL and Ovid Emcare. Studies were included if the research: a) was led by a nurse; b) described outcomes of a digital health intervention; and c) included any population with a prevalent chronic condition. FINDINGS: 26 studies were included in this review. Nurse-led digital health interventions are being used to support and manage a range of chronic conditions in varied settings. DISCUSSION: Digital health interventions are changing the delivery of healthcare for individuals living with chronic conditions. These interventions are bridging the gaps between the digital and physical worlds and are rapidly evolving. CONCLUSION: The recommendations posed in this review reiterate the importance of robust content and methods education for nurse scientists to address future research needs in a digital era.

RESUMO

Children, parents, older adults, and caregivers routinely use sensor technology as a source of health information and health monitoring. The purpose of this paper is to describe three exemplars of research that used a human-centered approach to engage participants in the development, design, and usability of interventions that integrate technology to promote health. The exemplars are based on current research studies that integrate sensor technology into pediatric, adult, and older adult populations living with a chronic health condition. Lessons learned and considerations for future studies are discussed. Nurses have successfully implemented interventions that use technology to improve health and detect, prevent, and manage diseases in children, families, individuals and communities. Nurses are key stakeholders to inform clinically relevant health monitoring that can support timely and personalized intervention and recommendations.

RESUMO

BACKGROUND: One of the critical components in pain management is the assessment of pain. Multidimensional measurement tools capture multiple aspects of a patient's pain experience but can be cumbersome to administer in busy clinical settings. AIM: We conducted a systematic review to identify brief multidimensional pain assessment tools that nurses can use in both ambulatory and acute care settings. METHODS: We searched PUBMED/MEDLINE, PsychInfo, and CINAHL databases from January 1977 through December 2019. Eligible English-language articles were systematically screened and data were extracted independently by two raters. Main outcomes included the number and types of domains captured by each instrument (e.g., sensory, impact on function, temporal components) and tool characteristics (e.g., administration time, validity) that may affect instrument uptake in practice. RESULTS: Our search identified eight multidimensional assessment tools, all of which measured sensory or affective qualities of pain and its impact on functioning. Most tools measured impact of pain on affective functioning, mood, or enjoyment of life. One tool used ecological momentary assessment via a web-based app to assess pain symptoms. Time to administer the varying tools ranged from less than 2 minutes to 10 minutes, and evidence of validity was reported for seven of the eight tools. CONCLUSIONS: Our review identified eight multidimensional pain measurement tools that nurses can use in ambulatory or acute care settings to capture patients' experience of pain. The most important element in selecting a multidimensional pain measure, though, is that one tool is selected that best fits the practice and is used consistently over time.

RESUMO

OBJECTIVES: to reflect on the principle of equity from the perspective of social justice and its applicability in the dimensions of the nursing work process. METHODS: theoretical essay on the challenges for the practice with equity in the dimensions of the Nursing work process: teaching, care, research, political participation and administration. RESULTS: the principle of equity should: in education be transversal in the training of future professionals; in assistance to occur without privilege or discrimination, treating the unequal unequally and the equals equally; in research be the target in the creation of new knowledge; in political participation to count on the contribution of social movements; and in the administration to count on the creation of specific executable policies. FINAL CONSIDERATIONS: there are still inequalities in access to services by part of the Brazilian population. Nursing should promote in its work process the implementation of equity promotion policies with a view to social justice.

RESUMO

OBJECTIVES: To analyze the social representations of nurses who work with assisted human reproduction about the operation with reproductive biotechnologies. METHODS: Qualitative approach, supported by the Theory of Social Representations, with sixteen participants. Individual, semi-structured interviews, analyzed through the Alceste software. RESULTS: Pragmatic elements related to nurses' performance from a professional, institutional, and public policy perspective in reproductive biotechnologies emerged, demonstrating the practical dimension of these representations. The characteristics of the professional to act in this area were addressed, showing the lack of information and search for scientificity; precarious perception of the organizational structure of health services; and attributions of nursing care arising from the health care practice in assisted human reproduction. FINAL CONSIDERATIONS: Social representation is anchored in the link between technology/medicalization and humanization/reception regarding reproductive biotechnologies. Working in assisted human reproduction involves a new and challenging nursing care, requiring specific and ethical knowledge.

RESUMO

Poor oral hygiene is an important risk factor for the development of non-ventilator hospital-associated pneumonia (NV-HAP), which imposes a significant burden on the NHS. This study aimed to establish whether the use of a 24-hour oral care kit and an oral care assessment tool can meet the needs of patients on an acute stroke unit. In comparison with the same period the preceding year, the introduction of the oral care kit and assessment tool improved compliance with oral care by more than 4 times; the overall costs of antibiotics to treat NV-HAP patients fell by 79%, with the number of doses falling by 70%, and the mortality rate decreased from 27% to 20%. According to a survey of multidisciplinary team (MDT) members keeping oral care tools available at the point of use saved time, and the oral health of patients on the unit improved after the introduction of the oral care kit and assessment tool. Almost all MDT members would recommend the use of the kit over previous interventions.
